York Teaching Hospital NHS Foundation Trust (YTHT) wishes to commission a service for condition monitoring for long term long term conditions e.g. Glaucoma and Macula and for patients requiring assessment including pre and post-operative follow up. The clinical decision making will remain the responsibility of YTHT. YTHT wishes to conduct pre market engagement to meet with potential suppliers to understand market capability. The scale of the potential service is as follows: Community Eye service. Glaucoma monitoring (9 months and above) York site estimated number of patients 1 000 — 1 400, East coast estimated number of patients 500-700. Dry AMD monitoring, York site estimated number of patients 300-400, East Coast estimated number of patient 150-200. New patient cataract appointment (consultant led), York site estimated number of patients 1 700 — 200, East Coast site estimated number of patients 850 — 1 000. Post operative cataract appointment, York site estimated number of patients 600 — 1 400, East Coast site estimated number of patients 300-700.
