Tender description
The aim of the Service is to provide high quality, responsive and clinically safe GP Out of Hours (OOHs) care accessible to the local population across Stafford and Surrounding villages, Cannock Chase and South East Staffordshire that supports system resilience across the health economy and improves patient experience and outcomes. The service forms an integral part of the emergency and urgent care system and enhances the provision of urgent care services provided in south Staffordshire. In addition the service will include out of hours care to prisoners within Staffordshire. Contract length will be for 36 months with the option to extend by 24 months at the discretion of the CCGs. Anticipated start April 2018. In addition the service will include out of hours care to prisoners within Staffordshire. The service will deliver the following objectives: — Improved pathways for patients accessing out of hours medical services and a seamless handover with in-hours primary care teams; — Provide a high quality, efficient and cost-effective service that fully meets the urgent primary medical care needs of patients; — Collaboration with all urgent care providers across the acute sector, community services, voluntary sector providers, general practice and other primary care providers; — Increased patient satisfaction, consistently reporting 95 % positive experience via the patient surveys; — Supporting the reduction of A&E attendances and emergency admissions; — Innovative use of IMT, ensuring appropriate access to patient records/patient special notes and systems to facilitate the sharing of information.
