Bracknell Forest Council's Adult Social Care, Health and Housing is looking to procure an outcome-based, flexible and sustainable Community Support Service (previously known as Domiciliary Care or Home Care service), that: — Supports older people and people with a long term condition to regain or attain independence to an optimum level, — Maximizes and maintains people to live independently for as long as possible, — Provides value for money and addresses the containment of costs. This drive to maximising independence for the people supported will require a significant cultural change both within the council, the individual and the provider market. In particular we are looking for Providers to partner with the voluntary and community sector creatively and flexibly to meet the needs of people. It is anticipated that through this approach, the level of care provision will reduce over the life of the contract.
