Pre Placement Agreement for the provision of Support to live in their Community for Adults with Disabilities

Tender description

The Service will be available for adults aged 18 or over who have a disability, and have been assessed as eligible for a service from Adult Social care (‘disability’ means a person aged 18 and over who is resident within the City of York who has been assessed by Adult Social Care as having a physical or mental impairment that has a ‘substantial’ and ‘long-term’ negative effect on their ability to do normal daily activities). The Service will provide care and/or support for customers of the Council’s Adults Services who have a disability. The services defined below may be delivered by one Provider or a variety of Providers to form the Customer’s package of support. All bidders must register their interest for the Yortender scheme titled “Pre Placement Agreement for the provision of Support to live in their Community for Adults with Disabilities”.
