Tender description

NHS Luton Clinical Commissioning Group and Luton Council are jointly commissioning an integrated health and wellbeing service that will work with our communities to proactively deliver a preventative approach that emphasises the whole person and which works to achieve the optimum levels of physical, mental, social and emotional health. The Luton wellbeing service will have one lead (Prime) provider who may work with local partner organisations under a single branding, promoting 'one-service', regardless of whether different organisations are delivering components of the service. The Luton Wellbeing service will be an integral part of Primary Care Home which is our Luton clinical model that enables the public to access wellbeing support in their locality. There is a requirement from commissioners that the prospective Lead provider will review how all Well-Being services can be redesigned to contribute to the delivery of constitutional targets within the financial resources available. The duration of the contract will be for a period of 60 months with an option to extend for a further 24 months up to a maximum 84 month contract duration. This is a notice for social and specific services in accordance with Directive 2014/24/EU of the European Parliament and of the Council Article 74; specifically Health and social services.
