Tender description

The provision of a 0-19 Wellbeing Model in Stockton on Tees. Commencing 1.4.2018 with a mobilisation period from contract award to commencement. The service comprises of three Lots which are: Lot 1 — 0-19 Healthy Child Programme; Lot 2 — Growing Well, Growing Healthy Service; Lot 3 — Family Outreach and Volunteers Service. This lot is an integrated 0-19 public health programme for children, young people and families, including mandated contacts. This lot is a high quality, and evidence based obesity prevention service that is outcome focused and can be embedded into school and community practices to achieve long term sustainable impact. This service includes National Child Measurement Programme. This lot requires delivery of a solution focused, Outreach Service in family homes which complements the support available through the Healthy Child Programme.
