Tender description

Kirklees Council wish to commission a Service which delivers and develops: — Dementia Friendly Communities — pre-diagnosis; — Information and Advice — post-diagnosis. The Dementia Information and Advice Service (DIAS) will work to raise awareness and understanding within communities in Kirklees pre diagnosis, signposting onto the diagnosis pathway where appropriate, and will offer all individuals being diagnosed with dementia access to a set of information and advice sessions. This Service will operate closely with South West Yorkshire Partnership NHS Foundation Trust who deliver the dementia diagnosis pathway across Kirklees. Tenders are invited by Kirklees Council from Providers with the relevant experience and ability to demonstrate sufficient capacity for the Dementia Information and Advice Service. 1 Contract will be awarded to a successful Provider, to deliver the full requirements of the Service Specification. Consortia bids are welcomed. It is anticipated that the Contract is to commence on 1.10.2017 and will expire on 30.9.2019, with an option to extend for a further period, or for successive periods, of up to 3 years. Tenderers should note that the contract sum, for delivering the requirements of this service, is fixed at 155 000 GBP per annum, refer to Clause 5 of the Terms and Conditions of Contract for further information.
