Tender description
Expressions of Interest for the Provision of a Community Based Anticoagulation Follow Up Service. NHS Manchester is seeking expressions of interest from organisations who can deliver a community based Anticoagulation service for patients registered with Manchester GPs. Anticoagulation services are currently provided by three acute trusts both within hospital and in the community. We are interested in providers who are able to deliver Anticoagulation therapy services at 3-4 locations within each of the PBC hubs (North, Central and South) and can demonstrate that they have the ability to offer the service to at least 1,500 patients. In order to be successful organisations should consider pricing advice contained in the Anticoagulation NES found at www.gpcontract.info/nes/index.htm (one patient attending on average for 6 contacts per year at level 4) At this stage the services will be delivered to a high proportion of patients who require lifelong monitoring. Potential providers will be aware of the likely introduction of drugs that may impact over time on this service. This will be reflected in the specification and contract length. The Anticoagulation services provided will: • Be provided from at least three to four locations in each of the PCT’s three PBC hubs. • Be offered to patients who have been assessed in a secondary care out patients and identified as stable within protocols agreed across primary and secondary care • Be offered to patients who have been commenced on or require the commencement of Warfarin or an equivalent for the treatment or prevention of thromboembolic disorders including atrial fibrillation, replacement heart valves, pulmonary embolism, deep vein thrombosis and peri-cardioversion. • Provide monitoring/long term monitoring with patients referred back to secondary care using agreed care pathways should their condition require it • Keep GPs informed of their patients’ condition and any changes.
