Tender description
NHS Southport & Formby CCG and NHS South Sefton CCGs are working collaboratively to procure a Community Anticoagulation Therapy Service. This is an invitation to attend a bidder information event. The overall aim of the service is to provide a consultant-led, community-based anticoagulation service which will improve patient access to initiation of anticoagulation therapy and provide monitoring of warfarin across various clinic sites, and in patients' homes for housebound patients. The service will be delivered to all appropriate patients registered with GP Practices in South Sefton and Southport & Formby and will ensure equitable access and quality of service to the entire population group. The service will be responsible for making specialist decisions regarding initiation of warfarin for patients referred into the service and provide ongoing monitoring and review of these patients. This will include regular reviews of TTR with recommended actions communicated to the patient's GP. In addition, the service will provide clinical expertise and input if changes to anticoagulant therapy is required (the service provider will manage any changes to therapy), implement bridging plans, and manage patients prescribed long term low molecular weight heparin (LMWH) therapy. This contract will be for an initial 3-year period with the option to extend for a further 1 x 2-year period. ** Prior registration for the event is essential ** The bidder information event will take place on Friday 14th July 2017 at NHS South Sefton CCG offices at 3rd Floor, Merton House, Bootle, Merseyside, L20 3DL from 10am. The event will consist of presentations by the CCG about the service which will be followed by a 1 to 1 meeting opportunity with members of the CCG. These meetings will be allocated on a first come, first come served basis and the CCG will make all endeavours that all interested parties are provided with a 1 to 1 meeting. Further information will be provided upon registration for the event. Bidders are asked to note that a maximum of 2 representatives per organisation will be permitted due to space restrictions.
