CTP809 Compassionate Communities Derbyshire

Tender description

The Compassionate Communities Derbyshire project aims to pilot a public health approach to end of life care in a maximum of 3 localities within Derbyshire (South Derbyshire, Erewash and High Peak). This project will support the delivery of the Ambitions for Palliative and End of Life Care Document published by National Palliative and End of Life Care Partnership and in particular Ambition 6 - Each Community is Prepared to Help. The 3 localities will develop an engaged community encouraging and developing practical support and the ability of all of us to discuss dying, death and bereavement in better-informed and more confident ways, will help dying and bereaved people to be better supported and ensure that they do not become isolated from their communities. We are seeking an organisation(s) to lead the project within a locality area to work with stakeholders, the local community, and recruit volunteers to deliver the aims and outcomes of the compassionate communities' project. The project will be in 3 lots • Lot 1 South Derbyshire • Lot 2 Erewash • Lot 3 High Peak (See Appendix I Locality Map) Organisations can bid for one or more Lots. The project must be completed by 31 March 2019, as the funding is non-recurrent and will cease after this date. The intension of the pilot project is to establish if this is a viable option for the communities of Derbyshire to deliver as a self-sustaining model.
