Tender description

City of York Council is re-commissioning its Adult Social Care (ASC) Reablement Service, which it out sourced in 2012, and invites suitably experienced providers to tender for the service. The Council's strategic direction is based on preventing, reducing and delaying demand for ASC support and our investment in Reablement is a key strand of this agenda. The Council is also engaging with NHS colleagues and the Voluntary and Community Sector to develop an integrated intermediate care service' for the City of York, and the Council's ASC social work teams and re-commissioned Reablement Service will be key elements of this more integrated service. The Council wishes to find a provider who is committed to partnership working and developing a flexible, dynamic and responsive service that can provide our customers with the right level of reablement support at the right time, helping them to regain skills and confidence and maintain their independence.
