Stop Smoking Service Level 1 (referral); Level 1 and Level 2 (intermediate smoking cessation); Level 1 and NRT provision (Pharmacist / dispenser); Level 1 and level 2 and NRT provision (Pharmacist / dispenser) and NRT Provision only (Pharmacist / Dispenser). Sexual Health Service (GP). LARC for Contraceptive Purposes. Chlamydia Screening and Treatment in Primary Care. Delivery of Integrated Sexual Health Services as a SH Sphere Clinic. Sexual Health Service (Pharmacy). The delivery of Emergency Hormonal Contraception, in line with local PGD arrangements. Screening 15-24 years National Chlamydia Screening Programme. Treating positive clients and partners for Chlamydia, in line with local PGD arrangements. Advice and referral in relation to longer term needs related to sexual and reproductive H&WB. Health Checks (GP). The NHS Health Check systematically offers those aged 40-74 years checks that assess risk of heart disease, stroke, diabetes, kidney disease and vascular dementia. Sexual Health Service (GP) GP providers who are able to work collaboratively with other providers in the Region, with a view to providing the following elements to their patients: — Long Acting Reversible Contraception (LARC) for Contraceptive Purposes (Specification SH01); — Chlamydia Screening and Treatment in Primary Care (Specification SH02); — Delivery of Integrated Sexual Health Services as a Sexual Health Sphere Clinic (Specification SH03). Sexual Health Service (Pharmacy) Pharmacies will work as part of a wider network of sexual and reproductive health providers, to deliver service to the populations of Bedford Borough and Central Bedfordshire: — The delivery of Emergency Hormonal Contraception, in line with local Patient Group Direction (PGD) arrangements; — Screening young people aged 15-24 years as part of the National Chlamydia Screening Programme; — Treating positive clients and attending partners for Chlamydia, in line with local Patient Group Direction (PGD) arrangements; — Providing advice and onward referral in relation to longer term needs related to sexual and reproductive health and wellbeing. Health Checks (GP) The NHS Health Check programme is a national initiative which systematically offers preventative checks to all those aged 40 — 74 years to assess their risk of heart disease, stroke, diabetes, kidney disease and vascular dementia. These diseases share a common set of risk factors: obesity, harmful alcohol intake, lack of physical activity, hypertension, smoking, disordered blood fat and/or sugar levels. Early identification and management of these risk factors can potentially delay or prevent the onset of vascular disease. Subsequently, reducing the potential burden on future health and social care.
