Tender description
Nottinghamshire County Council is seeking a suitably experienced provider / providers to deliver a dedicated Rapid Response and Hospital Discharge Service for Adults in Nottinghamshire (excluding Nottingham City) under a framework agreement. The Service is a short term, home based care service for people who are: — At risk of being unnecessarily admitted to hospital or short term care in a care home due to short term crisis at home or — Being discharged from hospital and who would benefit from a short period of a reabling or enabling type service to confirm or adjust their on-going package of care. The Provider and any sub-contractors must be appropriately registered with the Care Quality Commission (CQC). Using a competitive dialogue process, the Council will identify a Provider/s to deliver either a county-wide service, OR a service divided into Lots. The framework agreement will run for an initial period of 2 years. There is an option to extend for up to a further 2 years. The overall aims of the Service are to prevent unnecessary admission to hospital or short term care in a care home and to facilitate hospital discharges and help Service Users return home at the earliest opportunity in accordance with their assessed needs. The Service will offer rapid response, short-term home based care between the hours of 7:00 and 22:00. The Provider will deliver a range of tasks that support people to remain living at home as independently as possible. These tasks may vary according to the specific Service User's needs but will generally be of a social care nature and include providing support with all aspects of daily living, such as personal and domestic care. At times these could extend to include basic health care tasks. The overall aims of the Service are to prevent unnecessary admission to hospital or short term care in a care home and to facilitate hospital discharges and help Service Users return home at the earliest opportunity in accordance with their assessed needs. The Service will offer rapid response, short-term home based care between the hours of 7:00 and 22:00.
