Tender description

Provide domiciliary support to various service user aged 18 or over to maintain their existing skills and/or acquire new skills and to increase and maintain their independence in daily living routines. The service will also provide integrated high quality, patient-centred health care services based on the principles of clinical governance. The Provider will ensure that they work in partnership with the Council and the CCG to reduce unnecessary admissions to hospitals and care homes, support hospital discharge and provide integrated and preventative community services. The Provider will require a local base within the boundary of Leicestershire to ensure service delivery. The framework may be reopened during its term at the discretion of the Authority for service need and the number of framework providers for each Lot may be expanded when the framework is reopened. Provide domiciliary support to various Service Users aged 18 or over, usually those who reside in Leicester City and have been assessed as having eligible needs for the purposes of Adult Social Care or Continuing Health Care. The Service User profile may include, but not be limited to: older people;people who have dementia;people who may be physically frail;people with physical disabilities; people who are at the end of life; people with learning disabilities or other cognitive difficulties;people with mental health needs. To provide domiciliary support to various Service Users to maintain their existing skills and/or acquire new skills and to increase and maintain their independence in daily living routines. The service will also provide integrated high quality, patient-centred health care services based on the principles of clinical governance. The Provider will ensure that they work in partnership with the Council and the CCG to reduce unnecessary admissions to hospitals and care homes, support hospital discharge and provide integrated and preventative community services. Service Users of 18 years and above who are placed using this Lot will include but not be restricted to Service Users with: * Complex and enduring mental health needs. * A dual sensory impairment (e.g. visual and hearing loss). * Behavioural, emotional and social difficulties (BESD). * Severe physical impairment. These will be more complex cases, generally requiring a higher skill set of staff.
