Bath and North East Somerset (B&NES) Council, in conjunction with BaNES Clinical Commissioning Group (BaNES CCG) are considering the procurement of 5 short-term (e.g. 6-8 weeks per placement) ‘Discharge to Assess’ beds in a nursing home environment. Bath and North East Somerset (B&NES) Council, in conjunction with BaNES Clinical Commissioning Group (BaNES CCG) are considering the procurement of 5 short-term (e.g. 6-8 weeks per placement) ‘Discharge to Assess’ beds in a nursing home environment. This provision is to facilitate the rehabilitation and assessment of adults following hospital discharge and the identification of the most appropriate long term care service and accommodation setting. The anticipated client group is: patients who whilst medically optimised and suitable for hospital discharge, still require further ‘interim’ 24 hour care to allow rehabilitation and assessments to be made before decisions about their long term care needs are complete. This procurement supports the aims of the Home First model of Discharge to Assess in Bath and North East Somerset which seeks to support people in the most independent setting possible. It is expected that the service will be able to provide (subject to change and clarification in the ITT): — 5 nursing home beds. — Nursing and residential care provision in line with patients' needs and CQC requirements. — Provision of physiotherapy to aid rehabilitation and recovery which is delivered by a HCPC registered physiotherapist or physiotherapy assistant in their absence. — Access to an on-site gym to facilitate physiotherapy and rehabilitation. — Admissions accepted between 8:00 and 20:00, 7 days per week to facilitate same day discharge from an acute care setting. — The provider will accept a trusted assessor model or telephone assessment model to facilitate prompt handover of care. — The provider will support.
