Tender description
Organisations are invited to tender for the provision of a solution which will allow patients to monitor their health (as it relates to hypertension); allow patients to digitally upload their own health data to their GP record; encourage and support patients to improve self-care and ultimately lifestyle changes; release GP resources, such that they may dedicate more time to their caring and clinical activities.
