Tender description

The Authority wishes to award a contract for an Academic Evaluation of Healthy Choices Service, ("the Contract"). The Provider shall conduct a process and outcome evaluation of the Programme. This is to include the impact of the Programme on primary and secondary health outcomes post intervention at 12 weeks, 6 months and 12 months and provide commissioning recommendations.
