Tender description

NHSGG&C wishes to commission a service to provide support and advice services to people with profound and multiple learning disabilities, their families and carers. The service will also be open to all adults and young people transitioning from children's to adults services. People with profound and multiple learning disabilities experience significant health and social care needs, which in turn leads to the involvement of many health and social care providers. NHSGGC is committed to supporting people to achieve their full potential and therefore recognises that in order to achieve these goals and aspirations our services need to be responsive to the needs of people and make reasonably adjustment to the way we deliver them.
