The City of Cardiff Council seeks to appoint a single provider (or a lead provider within a consortium) to deliver domiciliary care within two ExtraCare accommodation schemes for Older People aged 60+ and younger adults with a physical disability in Cardiff. The City of Cardiff Council seeks to appoint a single provider (or a lead provider within a consortium) to deliver domiciliary care within two ExtraCare accommodation schemes for Older People aged 60+ and younger adults with a physical disability in Cardiff. ExtraCare combines the philosophy of care alongside housing related support services within a housing environment. The service aims to provide a person centred and planned approach to the provision of care and housing related support to the tenants of the scheme. Service user need for those in receipt of Extracare is based on a Unified Assessment and all people in receipt this service will have met Cardiff's defined eligibility criteria for Extracare.
