Tender description

SBC wishes to commission a service that enables customers and improves their wellbeing and quality of life. SBC is contracting for suitably experienced and qualified service providers capable of providing high quality Care and Support at Home services for individuals throughout the borough. This contract will be split into 5 lots: Lot 1 — Care and Support at Home (East, East Central, West and West Central Localities); Lot 2 — Home Again Service; Lot 3 — Community Support; Lot 4 — Spot Provider Framework; Lot 5 — Complex Needs Framework. For Lot 1 SBC is commissioning the provision of Care and Support at Home, including reablement; this will be split across the four Locality areas of Southend-on-Sea with one main provider commissioned to deliver reablement and longer-term care and support in each locality area. For Lot 2 SBC is commissioning a ‘Home Again Service’ to enable people to return home from hospital in a safe and planned way, reducing delayed discharge and readmission; the Council is seeking to secure one provider to support the delivery of a short-term, responsive Home Again Service to residents across the Southend-on-Sea borough where the contracted Locality Care and Support at Home Provider is unable to respond to the referral on the same day. For Lot 3 SBC is commissioning a Community Support Provider, where there is a specific support need that cannot be met by the Locality Provider, a contracted Community Support Provider will work across the borough to provide tailored support for people with a specific need including, but not limited to: — Adults with a learning disability, — Adults with a mental health need, — Adults / Children with an Autism spectrum disorder, — Children with a disability, — Children of adults with a physical / mental health / learning disability. For Lot 4, SBC is developing a Spot Provider Framework is aligned to the main Care and Support at Home contract; to provide a regulated framework of providers who are suitably experienced and skilled to provide care and support at home. This Framework will effectively be a list of approved providers that have demonstrated that they have the appropriate skills and experience to meet the care and support needs of adults, who will pick up any referrals that the main contracted Locality Provider is unable to take on. For Lot 5 SBC is developing a Complex Needs Framework which is aligned to the main Care and Support at Home Specification; ito provide a regulated framework of Providers who are suitably experienced and skilled to provide support to those people with a more complex support need; this may include, but is not limited to: — Children / Adults with a severe learning disability who have challenging behaviour, — Children / Adults with an Autism spectrum disorder, — Adults with a complex mental health need, — Adults with a complex health need who require rehabilitation.
