Tender description
Purpose Across Knowsley, Liverpool and Sefton (Tripartite) Adult Social Care is changing. By 2021 health and social care services across the Tripartite will be delivered around customer need and not organisational form. To achieve this, we will be: • Developing a Place Based Model of Care - Optimising community resilience, capacity and assets to support people in their own home. • Placing individuals and families at the heart of the care system, through self-directed models of care, to provide the most efficient and effective pathways across the system. • Focusing more on prevention and early intervention, maximising opportunities for self-care to keep people as independent as possible, for as long as possible. To support our ambitions, we are looking to: • Develop a strength/asset based Resource Allocation System (Lot 1) • Develop and implement a culture change programme across the Tripartite Social Care Workforce. (Lot 2) Scope of work Lot 1 – Development of a strength/asset based Resource Allocation System We are looking to the market for support to develop a resource allocation system (RAS). In supporting the development of the RAS, the successful provider will be required to undertake an audit across the Tripartite of the current and projected spend and budgets across Adult Social Care to fully understand the constraints in which the RAS must operate. The RAS must; • Ensure resources are fairly allocated based on need; • Capture information about what informal / unpaid support and assets can be harnessed to meet desirable outcomes; • Ensure sustainability of informal / unpaid support and assets; • Allocate resources to meet unmet outcomes • Be fully Care Act compliant • Be deliverable through the Authorities Care Mangement System (Liquid Logic). The key deliverables for the project will include: • A workable, Care Act compliant, resource allocation system that builds on good practice within the sub-region, and nationally and that can be implemented consistently across the Tripartite, with minimal reconfiguration of Liquid Logic systems. • A practice guidance document on utilising the Resource Allocation System. • A proposed implementation plan. Lot 2 - Develop and implement a culture change and practice development programme across the Tripartite Social Care Workforce. We believe that successful change and improvement in social care depends on the participation of a wide range of people. We are therefore looking to the market to support the redesign of assessment process through the co-production of a strength based / self-directed assessment and support planning approach for all client groups for adult social care across Knowsley, Liverpool and Sefton that: • Is person centred, seamless and proactive • Simplifies and aligns the current arrangement to make them user friendly to both individuals and practitioners • encourages individuals to make the best use of all their existing resources, abilities, skills, knowledge and social capital /assets. • encourages and supports people to experience new opportunities recognising any associated risks and managing these appropriately. The key deliverables for the project will include; • A strength/Asset based / self-directed assessment and support planning model for all client groups for adult social care across the Tripartite • A final report on the model and recommendations for implementation. • A workforce development programme that supports the required culture change and practice development needs of staff e.g. Asset Based Social Work & Creative Conversations, Managing Risk & Individual Choices • Provide practitioners and managers with the tools to encourage and develop strong, effective leaders who understand the purpose of the change and can play an active role in planning and achieving the change.
