Tender description
Full complement of hospice services, including: — Inpatient, outpatient and day care hospice services, — A community palliative care service, — A hospice at home service. Services are delivered by a multidisciplinary team (including doctors, nurses and HCAs) and provide holistic care and support for patients requiring specialist palliative care. St Raphael's is an SME and the only locally-based provider able to deliver these types of hospice services for Merton patients.
