Tender description

The London Borough of Hounslow (Authority) provides a care navigator and information and guidance (IAG) service for each of its localities. The service pro-actively engages/co-locate with 20 GP practices in the borough. The service is currently provided by Integrated Neurological Services (INS) at a value of 187 000 GBP pa. The Care Navigators will offer brief Intervention in the GP localities, so that residents can access the right support at the right time to maintain their independence for as long as possible. Offer referral and practical handover support to other partners including the Supporting Independence Service based in the Civic Centre, Hounslow. Provide IAG (Short term, practical, outcomes based support offered for a maximum of 8 weeks to realise short term goals. Support will typically focus on complex or specialist health and care issues until resolution or handover to a partner service). Develop close relationships with the Primary Care Practice Coordinators and contribute to Multi-disciplinary teams as relevant. Deliver drop in sessions in GP Surgeries.
