Tender description
This contract is for a 6 month extension to a Pilot scheme for those patients with a rapidly deteriorating condition that may be entering the terminal phase, the Fast Track process was set out in the National Framework to enable immediate provision of NHS Continuing Health Care. It is the CCGs intention to commission care that improves patient safety and quality of care for those with a life expectancy of less than 3 months. The extension is to allow for a formal tender to be undertaken.
