Short Description: This Community Connectors service is commissioned jointly by NHS Gloucestershire Clinical Commissioning Group (GCCG) & Gloucestershire County Council (GCC). The aim of the service is to increase the wellbeing & resilience of individuals and communities, & to reduce reliance on statutory services This new service supports the Gloucestershire Health & Wellbeing Board's Enabling Active Communities policy, by building upon the work of the current Social Prescribing & Village & Community Agents & joining both provisions into one new integrated service To access documents please register on our e-procurement portal: www.supplyingthesouthwest.org.uk Please ensure that any questions & clarifications are sent through this e-procurement portal.
