Tender description

NHS Manchester CCG wishes to commission a pilot proactive Primary Care and Pharmacy/Medicines Optimisation Support Service for care home residents within the North Manchester locality; the service will provide care home residents with the appropriate level of support that they require. There are 33 registered care homes in North Manchester with a registered population capacity of up to 1 096 residents (figures correct as at June 2016). Within the encompassing service there are 2 lots: Lot 1 is the Primary Care offer to Care Homes and Lot 2 is the Pharmacy/Medicines Optimisation offer to Care Homes. Both of these lots make up the service. It is anticipated that this will be a project delivered by either one provider delivering both lots or two providers bidding independently to collaboratively deliver the service to support the care gap for residents within the care home population in North Manchester. The service will then be rolled out to include all registered Care Homes (with the exclusion of identified learning disabilities homes until decision after 12 months review) so that by August 2018, all remaining care homes are included and integrated within the four North Neighbourhoods receiving the proposed service provision as part of One Team Place Based Care. This procurement will be run as a bespoke, single stage procurement procedure.
