Doncaster CCG would like to commission Clinical Assessment Beds across the Doncaster area from Nursing Home providers to facilitate patient transfer from an acute care bed to a complex assessment bed. The Successful Providers will accept direct referrals from acute Providers, and work in partnership with them to facilitate the transfer in to the complex assessment beds and then to co-ordinate the patients' discharge into their future place of residence. The Successful Providers will accept direct referrals from acute Providers, and work in partnership with them to facilitate the transfer in to the complex assessment beds and then to co-ordinate the patients' discharge into their future place of residence. A maximum of 29 beds will be commissioned across both Nursing and EMI Care Homes. Strategic objectives: — To provide a high quality, safe and personalised care home service that promotes choice, dignity, control and quality of life to all and ensure that care provided delivers a positive experience as well as the best patient outcomes. — To provide patient centred care in the context of the wider multidisciplinary team across primary, community (including specialist nursing teams), secondary and social care as well as the independent and voluntary sector. — To promote health and wellbeing and reduce health inequalities by offering pertinent advice and information as appropriate to service users, carers and other professionals. Where appropriate this may include sign posting to other agencies. — To ensure that assessment documentation is completed in a non-acute setting allowing the patient's independence to be optimised prior to Continuing Health Care assessment being undertaken — To deliver quality clinical outcomes, which achieve high levels of patient satisfaction which can be evidenced by patient and family questionnaires — To provide high quality and comprehensive nursing services to patients identifying incidents, concerns and complaints which promotes learning and is then embedded throughout the home.
