Tender description

Provision of Lot 1- Primary care and Occupational Health Services (on site). Lot 2- Gym Management Services (on site). Lot 3- Dental Services (off site). Duration - 3 years with option to extend for 2 x 1 year extensions. Lot 1 Medical Services - Primary Care and Occupational Health Services. Primary Care Practitioner, Occupational Health Practitioner and Nurse plus Medical Secretary. Duration of Contract - 36 month contract with the option to extend an additional 2 x 12 month extensions. Lot 2 Gym Management Services. To manage the in-house Gym facility and provide some external trainers to provide exercise classes and personal training services. Duration of Contract - 36 month contract with the option to extend an additional 2 x 12 month extensions. Lot 3 Off site Dental Services Provision of off-site dental services to compliment the on-site services already in place. No fixed number of patients will be agreed as part of the contract. Duration of Contract - 36 month contract with the option to extend an additional 2 x 12 month extensions.
