NOTE: This notice was updated on 09 March 2017 for the following reason: Additional service added, please refer to service specification for details including additional TUPE information and suggested Job Description for a Hospital Discharge Worker to coordinate the service. Consequently additional funding has been added which now totals: £46,008.75 from 1 July 2017 to 31 March 2018 and £61,345 from 1 April 2018 to 31 March 2019. Total funding for contract £107,353.75.Home from Hospital service operates as planned interventions and works in partnership with other service providers to prevent hospital admission through a range of support and interventions and where hospitalised, minimise disruption and ease the adjustment following a crisis or hospital stay, both for the patient and their formal carers. The range of services can be varied based on their local circumstances and the need of the individual
