Over the past 18 months Essex County Council has worked with partners to better understand dementia in Essex. We've listened to the voice of people living with dementia, their families, friends and wider networks. Through this work, we have identified opportunities to transform our approach, develop community support and improve outcomes for people living with dementia. We are now moving forward with our intention to re-procure Community Dementia Support in Essex. Essex County Council (ECC) are seeking a Provider to develop and deliver a service for people living with dementia and their families and carers across Essex. The service will work in partnership with professionals across the health and social care system in Essex, whilst taking into account local differences, to provide seamless access to coordinated information and advice to enable people living with dementia, and those that care for them to live well, and independent for as long as possible. The service will have 2 separate elements which link closely — the community support and a family information, support and navigation function. Initially there will be a connecting role, the Dementia Co-Ordinator who will prove the link between the elements, the community and local implementation.
