Tender description

The core aim of the service is to support families to live a healthy lifestyle, prevent the escalation of health and lifestyle related problems within our population and develop health and well-being action by communities, volunteers and front line services. The service will seek to improve a range of specific outcomes including: — Reducing childhood and family obesity. — Reducing tobacco use. — Reducing decayed, missing and filled teeth (DMFT) of children aged 5 years. — Reducing cardiovascular risk. — Reducing the impact lifestyle has on long term conditions. — Reducing health inequalities. — Increasing the uptake of other preventative services. The targeted interventions delivered by the service will be aimed at people: — With multiple lifestyle related risk factors such as high blood pressure, high cholesterol etc. — With disabilities that contribute to an increased likelihood of health risk factors. — With mental health problems and illnesses. — Who are obese and inactive. The service will seek to improve a range of specific Public Health outcomes including: — Reducing childhood and family obesity. — Reducing tobacco use. — Reducing decayed, missing and filled teeth (DMFT) of children aged 5 years. — Reducing cardiovascular risk. — Reducing the impact lifestyle has on long term conditions. — Reducing health inequalities. — Increasing the uptake of other preventative services. The duration of the contract will be for 3 years with the option to extend annually or a further 2 years.
