Warwickshire County Council is looking for a single provider or a consortia bid to provide county wide a short term Housing Related Floating Support service for Warwickshire Customers with disabilities, aged 16 and above. The Service will support them to stay in their own homes or work towards living in their own homes. In doing this the Provider will support Customers to prevent homelessness, hospitalisation or institutional care. In doing this the Provider will support Customers to prevent homelessness, hospitalisation or institutional care. The following partners will be able to spot-purchase housing related support services from the contracted Provider: — Warwick DC, — Stratford DC, — Rugby BC, — Nuneaton and Bedworth BC, — North Warwickshire BC.
