Tender description

NHS Scarborough & Ryedale CCG is looking to commission a multi-speciality community provider model for the delivery of a range of community based health and care services for its registered population. Whilst North Yorkshire County Council as a Commissioner of Services is not proposing to commission any social care services through this route at the outset it is fully committed to supporting the integrated care agenda and may, at the sole discretion of the Council, transfer a number of social care services and budget lines and functions to the chosen service provider during the life of the contract subject to Council approval and in accordance with the process set out in any resulting MCP contract.NHS England will be a co-commissioner in respect of any primary medical services delivered through this contractual route. It is anticipated that the provider will deliver some core services on from day 1 with the potential for additional services to be delivered from later points. Anticipated core services: — Community Services (Ryedale) — Community Services (Scarborough) — Community Specialist Nursing — Early Supported Discharge for stroke — Community Response Team (Ryedale Hub) — Community Response Team extension to Scarborough — Continuing Healthcare including Fast track — Funded Nursing Care Anticipated additional services from 1st April 2019 — elderly medicine out-patient services — fraility service — community diabetes — community outpatients e.g. rheumatology, ENT, dermatology, gynaecology Further services which may be required at a later point: — Adult and older people mental health and learning disability services — local enhanced services — primary care services (GMS/PMS/APMS) — Living Well prevention services — emergency duty team — day services — customer centre — re-ablement — extra-care housing — budgets for day to day management of home care, residential and nursing home care placements within framework Potential 10 + 5 year contract.
