Tender description

The London Borough of Hammersmith & Fulham, the Royal Borough of Kensington and Chelsea and Westminster City Council intend to jointly establish a Dynamic Purchasing System (DPS) for the provision of Extra Care Housing (ECH) across the three boroughs. The service delivers accommodation based care and support to enable people to develop or maintain their independence, minimise the risk of social exclusion and improve their quality of life, thus preventing loss of home or tenancy and significant reduction in the use of more institutional forms of care. It is possible that services under the DPS may be commissioned by other local authorities in addition to the three named here. There will be a separate category for each local authority to procure their ECH services. Each local authority may contract directly with a Service Provider of individual ECH schemes operating in their area. The Service is being procured under ‘Schedule 3 — Social and Other Specific Services’ of PCR 2015. Provision of accommodation based care and support to older residents in the borough. Promoting independence by supporting and encouraging residents to undertake their own self-care and daily living tasks as far as possible and as agreed on their support plan; —Support with personal care tasks;communication, social care and social integration as may be agreed; — Enabling residents to meet their tenant responsibilities to maintain their home, maintain safety and security, and access leisure activities; — Provision of domestic support not limited to cleaning, laundry, shopping, whilst promoting the residents' independence; — Access services and activities that promote mental and physical stimulation and social networks within the local community for the benefit of residents and those living in the surrounding area who may benefit from these opportunities; —Reduced personal and social isolation by developing their own social network, encouraging links with family, friends and community.
