Contract notice for Trauma and Orthopaedic Community Triage, Assessment and Treatment Service behalf of NHS Rushcliffe CCG. The service will consist of the following service elements: • Community triage, assessment and treatment lower limb clinic • Community triage, assessment and treatment upper limb clinic • Physiotherapy treatment course Aims and objectives of service • To ensure that clinicians are supported through appropriate guideline and protocols To make expert advice readily available to clinicians in primary care • Support the self-management agenda • To direct patients to the most appropriate service to meet their care needs • Reduce unnecessary hospital referrals • Increase number of patients managed in primary care and community settings where appropriate • Increase patient satisfaction • Improve relationships and collaboration between primary and secondary care • Make cost savings • Provide referral intelligence to assist on-going service redesign To express interest your organisation should register on the Bravo website https://ardengemcsu.bravosolution.co.uk/web/login.html to access the ITT Process Overview and ITT questionnaire.
