The service to be delivered is for the provision of care and support services within 3 Extra-Care housing schemes for older people, located in the borough of New Forest. The Extra-Care provision will be delivered within the borough of the New Forest, specifically in Totton, Lymington and New Milton. The service will provide the following support to service users: — Provide stability and consistent availability of care service provision, including at night, to meet eligible need. — Provide consistently good quality care. — Ensure individuals' safety, protection from harm, and social inclusion. — Work effectively with those seeking to access extra-care, scheme accommodation providers, the District and County Councils to facilitate effective and appropriate access. The contract will start on 30.10.2016 and will end on 31.3.2017.
