The Integrated 0-19 Healthy Child Programme Service will deliver an integrated approach to meet the needs of children, young people and their families and deliver improved outcomes. The Integrated 0-19 Healthy Child Programme Service will be a combined service of health visitors who work with children and their families aged 0-5, and school nurses who work with children and young people aged 5-19. In delivering the 0-19 Service, health visiting provision for the 0-5's will follow the Healthy Child Programme (HCP). For 5-19's, the 0-19 Service will be required to adapt the HCP to suit local need an capacity, but all children and young people, schools and other institutions who serve children and young people in this age group will have access to the Universal HCP to include signposting and advice. However it is expected that practitioners working in the 0-19 Service will be flexible in their response to need and in working with children and young people across the age range.
