Tender description

NNUH is looking for a hospital discharge management services, who would be contracted to work in liaison with the hospital discharge team and social care. The company would provide fully trained non clinical care co-coordinators to take referrals for patients who are medically fit and clinically stable for discharge, but who need to choose a care or residential home before being discharged. The patient and their family would be supported through the process of choosing and moving to a suitable care home with a dedicated and personalized care co-coordinator to accelerate the process. Procurement is looking for financial savings due to the reduction in ‘lost’ bed days — where the patient waits in an acute bed without an acute care need.
