North of England Commissioning Support (NECS) are working for and on behalf of NHS South Tees Clinical Commissioning Group (STCCG) who wish to explore the market to understand if a Provider is able to deliver a Community Assessment Unit (CAU), based within the Day Hospital Unit at Redcar Primary Care Hospital and accessible to patients registered with a South Tees GP practice. The CCG would like to commission a high quality, multi-disciplinary service designed to ensure that older people who are referred to the service have access to comprehensive geriatric assessment, led by either an Elderly Care Consultant or General Practitioner with Specialist Interest (GPwSI) in elderly care, receive a medical and / or social care diagnosis by a multi-disciplinary team (MDT), and are able to access treatment in the most appropriate setting without unnecessary delays, transfers of care or unnecessary admission to hospital. The service provider will deliver a Consultant and/or GPwSI led MDT frail and/or complex patient clinic 3 sessions per week, on a Monday, Wednesday and Friday mornings at Redcar Primary Care Hospital, seeing up to 6 patients per clinic. The number of appointments per clinic will be reviewed after 6 months to understand if there is capacity to increase this. The Multidisciplinary team will consist of: Consultant and/or GPwSI, Nurse, Health Care Assistant, Physiotherapist, Occupational Therapist and Social Worker. The service will aim to provide a coherent, multi-disciplinary service designed to deliver care across traditional organisational boundaries that supports and adds value to patient care from General Practice and Primary Care and utilises community and acute provision to ensure patient needs are managed in a proactive, anticipatory, holistic and integrated way. The providers will organise themselves to collaborate, cooperate and coordinate to achieve shared goals and to support the achievement of the best possible outcomes for patients. NECS is utilising an electronic tendering tool to manage this procurement and communicate with potential providers.
