SCC IC Child and Young People (5-19 years) and family lifestyle weight management service

Tender description

This is a notice for social and specific services in accordance with Directive 2014/24/EU Article 74. Surrey County Council are seeking to commission provider(s) to design and deliver an accessible tier 2 lifestyle weight management service(s) for children / young people between 5 and 19 years of age who are overweight or obese and their families to help them develop healthier lifestyles and reach and maintain a healthier BMI. The service will form an integrated part of the planned local healthy weight / weight management care pathway. The provider(s) will be required to deliver a tier 2 weight management service for children/young people and their families in each of our five CCG areas across Surrey. This will be a two year contract(s) with the option of a one year extension.
