Tender description
The national Healthcare Public Health team in Public Health England (PHE) is leading a project to support the commissioning and provision of health and care services working together as systems in England, using two areas as exemplars, atrial fibrillation (AF) and falls and fragility fractures (FFF). This project will support PHE in delivering its remit of improving population health through sustainable health and care systems. PHE is seeking a provider to support the delivery of this project.
