Tender description

Sheffield requires a comprehensive Mental Health Liaison service, integrated across the whole health economy operating 24 hours 7 days a week with staff from a wide skill base, with opportunities to work in close association with senior psychological therapists, specialist liaison mental health nursing, specialist psychotherapy, counsellors, co-ordinators, advocacy services, occupational therapists, physiotherapists and the voluntary sector. The service will comprise a specialised, fully integrated team comprising multi-professional health care staff, under single leadership and management, working closely with the acute hospital and Primary Care It will offer all-age inclusive services, including liaison services for children, older people and adults with dementia, targeted at reducing avoidable admissions, particularly for people with complex multiple morbidity including people with personality disorder, borderline personality disorder, learning disability, dementia, autism and Medically Unexplained Symptoms. The service will engender transformation of physical and mental health service integration, to continue to move us towards Parity of Esteem. It will ensure effective liaison with other parts of the health system, including general practice, crisis and in-patient teams, specialist mental health teams, social services, emergency services and non-statutory agencies. The service will deliver supervision, education, liaison and direct clinical activity outside the acute setting and into primary care when care pathways for patients with MUS, LTCs or other issues require consistency of care in order to avoid deterioration or re-admission. The service will offer specialist consultancy advice and training to all working within the multi-disciplinary comprehensive liaison model in order to support people with complex multiple needs. This will reduce referrals and investigations as well as A&E attendances. The new service will enable the integration of adult liaison services with the liaison services provided by Sheffield Children's Hospital, particularly for those young people who are supported through CAMHS services, to complement the development of the planned CAMHS Tier 3.5 service.
