Tender description

Tenders are invited for the provision of a care support and welfare navigation service This Service is provided to meet the Council’s requirements in relation to the National Stroke Strategy to help individuals and carers and develop support networks within the wider community. The Service will consist of care, support & welfare navigation services and will include but not be limited to: • Care, support and welfare navigation • Timely and appropriate information and advice relating to stroke • Support with discharge planning and/or transfer of care • Sign posting particularly to ongoing support from relevant providers of care • One-off intervention/advice • Generic support • Individual practical and emotional support for stroke survivors • Access to well-established carer support groups • Liaison and joint working with key agencies to meet needs of Client, Carers and other family • Stroke prevention advice aimed at encouraging lifestyle changes • Access to welfare grants • Access to other rehabilitation and support services • Providing training sessions for carers of stroke survivors.
