Tender description
Block Contract for Home Care in Contract Areas C2, F2 and F3. (3 Contracts) The aim of the Service is to support Individuals who require care and support to help them live in their own homes. The model of Service will be 1 that enables people to achieve as much independence as they can by working with them to make the most of their skills and abilities in relation to their physical, intellectual, emotional and social capacity. The Council intends to move away from the traditional delivery model of support to a Service which is Outcome-Focused. The Provider will offer a range of solutions to support an Individual's care and support needs as described in the Individual's Support Plan. The Service will be delivered in the Individual's own home and in their local community in accordance with the Specification.
