Tender description

‘Stockport Council is procuring a new preventative service for adults of all ages called the Targeted Prevention Alliance (TPA). The Council welcomes bids from alliances of providers who will jointly and collaboratively deliver the Targeted Prevention function as a single service. It is estimated that the value of this contract will be up to 1 500 000 GBP annually. This is a strategically important service within the Stockport social care and health economy as it will support the implementation of the Care Act and the evolving model of integrated health and social care. The TPA will offer a range of support and services to people with a diverse range of needs and must aim to encourage all service users to be more resilient and able to better advocate for themselves, as well as promote the need for a positive relationship with their wider community. The service must pro-actively target and be open to supporting any person struggling with 1 or more aspects of independent living, regardless of the reason for this. It will offer an increasingly more intensive form of support, which is proportionate to people's needs, ranging from information, advice and signposting to a more assertive and proactive type of support centred around an Individual Support Plan.’
