Tender description
These services will work with individuals, families and communities to improve the health and wellbeing of local populations by assessing and responding to local health needs. The service will deliver the national Health Visiting model which includes 4 levels of service; five universal reviews and six high impact areas. Registered Health Visitors will lead skill mix teams and coordinate delivery of the universal “Healthy Child Programme Pregnancy - 5 years”
