Tender description

This is a notice for social services in accordance with Directive 2014/24/EU of the European Parliament and of the Council Article 74. Type of services — Social and Other Services. Joint Commissioning of Community Based Domiciliary Care and Support in Leicestershire County — The Help to Live At Home Service (HTLAH). Leicestershire County Council, working jointly with West Leicestershire Clinical Commissioning Group and East Leicestershire and Rutland Clinical Commissioning Group intend to commission new home-based care and support services to meet the health and social care needs of service users and carers across the county of Leicestershire. Service users will be individuals aged over 18 who have been assessed as eligible to receive services commissioned either through the local authority or assessed by health under continuing healthcare (CHC). One of the commissioners' key strategic intentions is to support people to live independently in their own homes for as long as possible. In order to do this, the commissioners require a stable and robust market of providers who have an experienced, motivated and well-trained workforce who are able to deliver a high volume of affordable, consistent and high quality services. The requirement includes a wide range of flexible services for people with varying levels of need which will include the delivery of associated social care and healthcare tasks. The aims of the service are to enable service users: — to safely remain in their own home; — to achieve their maximum potential through delivery of “step up” community reablement service; — to optimise and maintain their independence by ensuring that reablement remains central to service delivery. The service will also facilitate people's safe and timely discharge from hospital and reduce avoidable hospital admissions. It is anticipated that a single service provider will be appointed for each of the 18 lots; with a restriction on any 1 service provider being awarded more than 3 lots. Lots are as follows: Lot 1 — Castle Donington and Whitwick; Lot 2 — Ibstock and Measham; Lot 3 — Ashby-de-la Zouch and Coalville; Lot 4 — Loughborough East; Lot 5 — Loughborough West and Shepshed; Lot 6 — Melton Mowbray; Lot 7 — Market Harborough; Lot 8 — Thurnby and Syston; Lot 9 — Oadby; Lot 10 — Wigston and South Wigston; Lot 11 — Blaby and Countesthorpe; Lot 12 — Hinckley and Twycross; Lot 13 — Groby and Market Bosworth; Lot 14 — Broughton Astley and Burbage; Lot 15 — Glenfield and Braunstone Town; Lot 16 — Narborough and Lutterworth; Lot 17 — Birstall and Anstey; Lot 18 — Mountsorrel and Quorn.
