Tender description

The Council are intending to award a contract to Partnership of Care and Allied Healthcare for the Provision of Domiciliary Care on behalf of Social Services. The purpose of this service is to provide specialist Domiciliary Care to all client groups within the borough of Caerphilly. The Council is committed to delivering a flexible and person centred approach to the provision of domiciliary care in line with the Social Services and Wellbeing Act 2014. The aim of this service is to: Provide reliable and consistent practical support that helps people increase control over their lives and remain independent. Prevent admission to residential or nursing home care whilst ensuring that Service Users are able to live, and access services, within the community. Assist carers to continue with their caring role. Enable discharge from hospital to take place appropriately, safely and without delay. Ensure that timely and appropriate support is given, taking into account Service User culture and preferences. Ensure that support and assistance is provided in such a way that promotes independence and manages risk effectively. Achieve agreed outcomes for Service Users. Ensure that transfer of care to another care provider takes place and a safe and planned manner.
