Tender description

The Social Wellbeing service will improve the health and well-being in the district. The Service will have 2 key priorities; to increase health and well-being and promote self-management. The Service will deliver the following direct measurable outcomes to improve health and well-being in the local population as a whole: — Self-reported health and well-being is improved. — People are equipped with the confidence and tools to have a good life. — Knowledge and uptake of services to increase well-being, self-management and independence is increased. — People feel less isolated and have more opportunities for social interaction and participation. — People stay independent for longer due to increased access to information and better coordinated services. The Social Well-being Service will improve the health and well-being of the district. The Service will have 2 key priorities; to increase health and well-being and promote self-management. The Service will commence on 1.4.2017 and will be put in place for 4 (four) years; therefore the expiry date will be 31.3.2021. The Council may extend the Contract by up to 24 months, making the final possible expiry date 31.3.2023.
