Tender description

There will always be situations where it is necessary for people to be admitted for long periods within an Acute setting. However, for those who do not require long term admission but still require care should, where it is clinically appropriate, be discharged to a more appropriate care pathway. Part of the National Emergency Care Improvement Plan (ECIP) identifies a requirement for local healthcare systems to implement a “discharge to assess” (D2A) model, in order to allow health and social care assessments to take place in a patients’ normal place of residence or a nursing home.
